Do you accept my medical insurance?

4th & Goal Athletic Training uses a direct-pay business format and insurance is not billed. Payment is required at the time of service and you will be issued a receipt for treatment provided. It is our goal to save you time and money by providing exceptional treatment so that you are on your way quickly. 4th & Goal strives to deliver 1 on 1 sessions with advanced skill that are more affordable than other organizations that often offer a cookie cutter approach to every condition.

Why should I choose to visit a Direct-Pay business over traditional big hospital care?

In the era of high-deductible health care health care plans, many people will pay exorbitant prices for each service until their deductible is met and then will be on the hook for a certain % of the uncovered amount after that. For patients/clients who are self-motivated to put in the work with only a few treatments needed, this format may save them considerable time and money. Direct pay offers patients:
Improved Access with shorter wait times for care
Personalized Care- 1 on 1 personalized sessions with longer appointment times, which equals fewer sessions (2-6) and more efficient outcomes.

Freedom– your practitioner has the freedom to treat you in a clinically relevant way without having insurance companies dictate the care you receive and what portions of the treatment they will cover.

Cancellation Policy?
Appointment slots are limited each week, so in the event that you must cancel-please do so at least 24 hours prior to your appointment. Cancellations within a 24-hour window of your appointment will incur a $50 cancellation fee.

What forms of payment do you accept?

Visa, Mastercard, Discover, AMEX, or cash. FSA/HSA- coming soon! For health/flex savings accounts a detailed receipt will be issued to you

What should I bring to my appointments?
We are here to help you with your athletic injury or for exercise prescription, so please wear athletic clothing- shorts, T-shirt, athletic shoes. If you are coming in for Dry Needling of the Upper extremity, please bring a tank top or similar. For CPR courses, dress comfortably but also with apparel that allows you to get up and down from the floor with ease.
